We're discussing Medic to EMT-CC downgrade on ALS call?
Re: Medic to EMT-CC downgrade on ALS call?Go to Top
Mr Schajer, Regarding the Issue with the EAB having CC overseeing medics: The PD EAB supervisors do not give orders to AMT's on patient care (99%) of the time.. The only time you would see an EAB supervisor giving a patient care order to an AMT is if the AMT was doing something that would harm a patient.. The FDNY also has Lieutenant's and Captains that oversee medics. Same is true for many private agencies.

Not sure if it has been mentioned yet, I only read through a handful of the posts, but I know if a medic does something that is only in their standing orders they can not hand that patient off to a cc.

Mr Schajer...Nassau County is not the only county that does not see the difference between EMT-P and EMT-CC or NREMT-I-99 (called Cardiac Rescue Technician in Maryland)..
Many MD depts. do not diferentiate, a "Medic Unit" can be a P or I-99,,, so do not get down on Nassau about that.
VA as well has some mensa medic that is also an off shoot of I-99, forget what they call it.
As for CC's that do not recognize the difference, (and I hate to defend them but) as a former CC that is now a P for many years, it is hard to understand unless you've been thru the 2 courses. And I will say, some CC's, are more knowledgable than some P's, but in general the level of training and knowledge for a P is vastly superior to a CC. IMHO. So even if the protocols were IDENTICAL, a P that handed off a patient to a CC, runs the risk of the lesser trained person, not recognizing some sign, symptom etc, and potential harm coming to the patient.

Hey EMSteach, the scary thing is that I have been told that exact phrase by some CC's that believe it.
I have written to Nassau Remsco that as of this post, has ignored my written request to give an opinion. Even a response would have been appreciated. I guess if they bury their heads in the sand, they wont see what is going on.
Unfortunately, this may come to a head through litigation and some unfortunate EMT-P will suffer because there is no official policy. There isnt even an advisory opinion which, although not the law, would give a little bit of crutch to lean on during a civil suit. Best bet, dont downgrade to a cc unless, as previously stated, it could be downgraded to an EMT.
Re: Medic to EMT-CC downgrade on ALS call?Go to Top
I finally did receive a response from Nassau Remsco. There position is clear: The hightest medically trained, (EMT-p, CC or EMT) at the scene is responsible for the patient care. If the highest care, the paramedic turns the patient over to a cc, and an untoward event occurs, the highest level is still responsible.

And with that, I think this thread has run it's course. Before there are any other issues and a CC vs P war, it's done..
