LONG overdue. Like the asthma protocol here for BLS, vs. in NYC. NYC EMT's were assisting with Nitro when Nassau ALS was assisting with Nitro. It seems Nassau as a region is always the last.
Yesterday, ReMAC approved the use of Nitro for CC's on standing orders for the Chest Pain protocol.
No change to the remainder of the protocol, pt. still needs systolic of 120 w/ no IV access or 100 w/ an IV.
We've been asking for this change for some time, glad to hear it finally came about. Now, if I can just get Benadryl on standing orders.....sigh....
So...now whats the differeance between CC's and Medics, it seems as if you are going to have to revise the long standing and famous comparison and perhaps say it with less anger, you are no longer "just medics without Nitro and Lasix on standing orders"...

Now Jeep, you've known me for how many years??? You have to get the line straight....and it's only funny when said w/ a certain NSUH medic in the room...the comeback is "the only difference between CC's and a medic is 8 hours of cardiology and two more drugs".....to this day it gets a rise out of him. I swear I say it just to see the vein in the middle of his forehead stand out. On a good day you can see the vein throb!!! LOL!!!!
What about the draft protocols that were sent out not too long ago. Not sure, but was this one of them?

No Sig12, it's not that it seems that Nassau is last...Nassau IS last. When we pushed for the Albuterol protocol, we literally plagerized (sp?) NYC's protocol, put in on Nassau REMSCO letterhead, and the docs out here rejected it. The screwed up part was that the ReMAC chair at that time was one of the docs from NYC that created the policy. Go figure.
Our biggest challenge right now in advancing Nassau's system is that we still, to this day, don't have a full time medical director. In the absence of Suozzi or Gulotta ever creating a budget line to pay a doc to be medical director, and in the absence of anyone at 1 West St. caring about the system, the REMSCO has, by it's operating authority appointed the current ReMAC chairman as the Acting Medical Director.
Our system is at critical mass. We need to make huge overhaul changes, and now.
That being said, I'm still pleased that we got Nitro. Like I said, all I need now is Benadryl on standing orders and I'll be happy. As a -CC, I can't give it on standing orders for anaphalaxis. But we can stop on the way to the hospital and the patient can run into Walgreens and buy a box OTC.

Ya know, just because a guy has a near death experience and has to have his own VAC safe his life doesn't mean you should tease him about it. After all, I am a senstive guy....

Haha, yeah, I learned CPR in your living room, a questionable Hispanic babysitter was the only other person in my "class". That was like 7 years ago now....we won't talk about the other occupants of that appartment...

Wasn't too long ago ... 1999/2000, EMT's were carrying their own albuterol in NYC, while AEMT's were asking for an order to use it in Nassau. Teach like you said - mirror image protocols, yet the County has to trail behind.
Any idea when they are going to put pen to paper and actually notify of the Nitro protocol change? I know its asking too much for someone at REMAC to actually send out a notification within a reasonable amount of time..lol
